Any form can be:

How Forms Work

1. Build or Customize a Form

Admins open the Form Builder and:

  • drag in required fields

  • create logic (if “surgery,” show pre-op questions)

  • add signature blocks

  • set automations

  • save as a template


2. Send or Automate

Forms can be:

  • triggered automatically (booking, reminder, surgery)

  • sent on demand from MsgBoard

  • embedded in MyPet intake

  • added to a case workflow


3. Client Completes the Form

Client opens the secure link:

  • answers questions

  • reviews clinic policies

  • signs electronically

  • submits

ClinicOS tags the form with:

  • timestamp

  • IP/device metadata

  • client identity

  • patient identity


4. Form Syncs to ClinicOS

Once submitted:

  • data populates the patient record

  • files attach to the visit
