How Forms Work
1. Build or Customize a Form
Admins open the Form Builder and:
drag in required fields
create logic (if “surgery,” show pre-op questions)
add signature blocks
set automations
save as a template
2. Send or Automate
Forms can be:
triggered automatically (booking, reminder, surgery)
sent on demand from MsgBoard
embedded in MyPet intake
added to a case workflow
3. Client Completes the Form
Client opens the secure link:
answers questions
reviews clinic policies
signs electronically
submits
ClinicOS tags the form with:

4. Form Syncs to ClinicOS
